The Intersection Where Anti-Racism and Healthcare Meet

An Ecosystem of Care for Children with Complex Health Needs

Integrated health systems improve equity, quality, and outcomes compared to traditional healthcare settings, while reducing costs through coordinated, comprehensive, and family-centered care.

Ecosystems of care work through a combination of social care systems, community organizations, government agencies, and community leaders. These organizations collaborate to design and implement programs, systems, and policies that effectively address the health needs of their communities.

This model is an effective way to connect health systems with important social determinants of health including housing, transportation, food access, behavioral health, and other sectors. The result is healthcare that focuses on all the elements involved in the health of the child.

Additional outcomes of successful ecosystems of care include:

  • Decreased healthcare worker burnout
  • Improved health equity
  • Better quality of care
  • Reduced health system costs
