The Camden Coalition found that single care management programs do not adequately serve patients with complex social and health needs and hope to bridge the gap with the use of their Ecosystem Assessment Tool. Relying on other sectors is a critical part of a thriving health network, and each part—workforce, services, leadership and governance, and consumer participation—should work together to provide accessible healthcare in the community. Partnerships with local organizations, engaging people with lived experiences, and diverse workforces, are steppingstones to improvements within healthcare.
Ecosystems of care have several key benefits that can make a difference in communities such as:
- Pinpointing areas of success, improvement, and development
- Creating a commonality to maximize efforts
- Ensuring equitable practices are implemented and upheld in all aspects of work
