Improving care coordination for people enrolled in both Medicare and Medicaid can help eliminate issues with healthcare delivery and improve patient outcomes.

Individuals enrolled in both Medicare and Medicaid, deserve the opportunity to easily navigate healthcare systems and access care. Too often, they face barriers to accessing high-quality services, high out-of-pocket costs, and a lack of help navigating the programs. Some find their eligibility restricted because of where they live.

The following recommendations can improve program navigation and support better health outcomes:

  • Simplify enrollment across the two programs to allow easier access to culturally responsive care.
  • Meaningfully engage people enrolled in the programs and provide opportunities for them to share their experiences to inform policy and program design.
  • Eliminate payment barriers by ensuring clinicians are reimbursed for the services they provide and reinvest savings from less costly services back into Medicaid programs.
  • Combine Medicare and Medicaid data to better report gaps in care.
A national review of 35 non-governmental public health and community power-building organizations (CBPOs) uncovered opportunities to strengthen their partnerships and mutual support to improve health equity efforts in communities.

The report explored gaps between public health and CPBOs and identified avenues for collaboration through advocacy and policy efforts. Despite cultural divides, the findings revealed opportunities to work together to achieve racial justice and improve health equity.

  • Public health NGOs can identify campaigns they are willing to align with, beginning with building relationships with CPBOs and engaging in advocacy to support those campaigns.
  • Public health academics can set up “activist labs” modeled on those already in place and implement advocacy and community power-building into their curricula.
  • Funders can support initiatives at the local and national levels, incorporating community power-building strategies into their theories of change and funding streams.
  • CPBOs can reach out to public health counterparts and invite them to engage in advocacy opportunities that begin to build trust and relationships.
In the absence of specific, standardized laws, financial assistance programs at nonprofit hospitals fail to provide financial relief to those who need it.

Currently, federal law does not set standards on the amount of financial assistance hospitals must provide, or who qualifies for help. Patients are often unaware of financial assistance programs despite existing laws requiring hospitals to post information about them.

To ensure equal access to financial assistance, states can set standards for how hospitals…

  • Determine eligibility
  • Define the amount
  • Simplify applications
  • Inform patients
  • Use predictive tools
Integrated health systems improve equity, quality, and outcomes compared to traditional healthcare settings, while reducing costs through coordinated, comprehensive, and family-centered care.

Ecosystems of care work through a combination of social care systems, community organizations, government agencies, and community leaders. These organizations collaborate to design and implement programs, systems, and policies that effectively address the health needs of their communities.

This model is an effective way to connect health systems with important social determinants of health including housing, transportation, food access, behavioral health, and other sectors. The result is healthcare that focuses on all the elements involved in the health of the child.

Additional outcomes of successful ecosystems of care include:

  • Decreased healthcare worker burnout
  • Improved health equity
  • Better quality of care
  • Reduced health system costs
